Welcome to the Spine & Wellness Centre new patient connection!

Here you will find the following documents – click to download:

• New Patient Welcome Packet which includes practice information, a copy of the practice policies, pricing and other helpful information.

• New Patient intake forms which we would like you to complete and bring along with you to the appointment (signature required on the 5th page).

We also encourage you to view the New Patient area on our website, you’ll find this information within the three menu tabs above this one under ‘New Patients’. This will give you details on what to expect upon your first appointment as well as the follow-up Report of Findings visit and ongoing care and payment information. 

A few extra notes…

Please note that your cost will be £60 (normally £130).  This fee will cover your first visit (initial comprehensive exam, approximately one hour in duration), your second visit (report of findings, approximately one hour in duration) and your first treatment which will either be on the second or third visit. 

Please kindly note that a standard £1 is charged for all card transactions.

We kindly ask that you arrive 15 minutes early for your appointment; this will allow us time to check the paperwork and answer any questions you may have before you start.

Due to the nature of the examination we kindly ask that you arrive in gym style clothing.

Lastly, if you need to change your appointment we kindly request a 24 hour notice.
